Printable Paperwork

When coming to Synergy for your first treatment we require that you fill out some paperwork including a health intake form, and policies & authorizations. For your conveinence you can download, print, and fill out the required paperwork to bring it with you.

Policies

ARRIVAL

When coming for you first treatment at Synergy please arrive 15 minutes early to insure enough time to fill out paperwork. If you prefer you can bring completed paperwork with you (see "Printable Paperwork" to download documents).

 

"IN SERVICE" SIGN

We are a small establishment and therefor may have the door locked while we are in service with a client. If you happen to arrive early to a sign reading "In Service" it will only be a few minutes before the door is unlocked or you.

 

PARKING

There is street parking in front of our building, as well as on the side street (2nd street).

 

CANCELLATIONS

If you are unable to make your appointment we require a minimum of 24 hours notice, with the exception being an emergency.  

 

All missed appointments without the proper notice will automatically be charged a flat rate of $40.00, and treatment will not resume until payment is made by the individual. Your insurance company is not respnsible for this charge and therefor will not pay.

 

LATE ARRIVALS

Please be respectful of your therapist's time and arrive accordingly. If you are late for your scheduled appointment you may find your treatment is shortened as to not affect other individuals scheduled after you.

 

GIFT CARDS

Gift cards are available for purchase at our phyiscal location for any denomination you prefer.
